POSITION OVERVIEW
We are seeking a licensed California physician for a per diem engagement to serve as Supervising Physician and Medical Director in partnership with our Management Services Organization (MSO). This role is central to our telehealth-based chronic care management practice and requires a physician committed to taking full ownership of the Professional Corporation (PC) to ensure complete compliance with California's Corporate Practice of Medicine (CPOM) laws and applicable regulations.
Our primary clinical services are pulmonary rehabilitation and cardiac rehabilitation delivered through a telehealth model. The ideal candidate brings deep clinical expertise in these areas, a thorough understanding of the PC-MSO operating structure, and the professional maturity to lead a compliant, high-quality practice.
KEY RESPONSIBILITIES
Medical Direction & PC Ownership
- Serve as the sole or primary owning physician of the Professional Corporation (PC) in compliance with California CPOM statutes
- Partner collaboratively with the MSO to establish and oversee administrative, operational, and billing infrastructure under the PC-MSO model
- Ensure all clinical operations and services rendered under the PC adhere to California Medical Board standards, CPOM guidelines, and applicable federal regulations
- Assume medical-legal responsibility for the PC's clinical activities, including review and co-signature of clinical protocols, standing orders, and policies as required
- Participate in periodic leadership meetings with MSO management to align on compliance, quality improvement, and strategic goals
Supervision & Clinical Oversight
- Supervise non-physician clinical staff including nurse practitioners, physician assistants, respiratory therapists, and other licensed providers as permitted under California law
- Review, approve, and sign standardized procedures and collaborative practice agreements as required for mid-level practitioner supervision
- Provide clinical guidance, medical oversight, and consultation for patient cases escalated by clinical staff
- Review and authorize clinical documentation, care plans, and patient records in accordance with established protocols and payer requirements
Telehealth & Chronic Care Management
- Support telehealth-delivered pulmonary and cardiac rehabilitation programs as the responsible supervising physician
- Oversee chronic care management (CCM) services, including participation in care coordination workflows as clinically and regulatorily required
- Remain current on CMS and California telehealth regulations as they apply to pulmonary and cardiac rehab reimbursement and clinical protocols
Credentialing & Payer Relations
- Maintain an active and accessible CAQH ProView profile and provide timely CAQH access to support the organization's provider credentialing and re-credentialing processes
- Cooperate fully with the MSO credentialing team to complete payer enrollment applications accurately and on schedule
- Maintain current in-network status with one or more of the following preferred health plans: Aetna, Cigna, Anthem Blue Cross Blue Shield (BCBS), and/or Kaiser Permanente
- Notify the MSO promptly of any changes to licensure, malpractice coverage, or payer network status
MINIMUM QUALIFICATIONS
- Active, unrestricted California Medical License (MD or DO) in good standing with the Medical Board of California
- Board certification in Pulmonology, Internal Medicine, Cardiology, or a closely related specialty; additional training in pulmonary/cardiac rehabilitation strongly preferred
- Minimum 3-5 years of clinical experience in pulmonary care, cardiac care, or pulmonary/cardiac rehabilitation
- Demonstrated experience practicing within or overseeing a PC-MSO structure with a thorough understanding of CPOM compliance obligations
- Established experience providing or supervising telehealth services in California
- Active CAQH ProView profile with the ability to provide authorized access to the MSO for credentialing purposes
- Active professional liability (malpractice) insurance with appropriate limits
PREFERRED QUALIFICATIONS
- Current in-network participation with one or more of the following payers: Aetna, Cigna, Anthem Blue Cross Blue Shield (BCBS), and/or Kaiser Permanente
- Prior experience serving as Medical Director or Supervising Physician for a telehealth or value-based care organization
- Familiarity with CMS chronic care management (CCM) billing codes (99490, 99491, 99439, etc.) and documentation standards
- Experience with remote patient monitoring (RPM) or principal care management (PCM) programs
- Knowledge of California AB 890 regulations governing nurse practitioner practice and supervision requirements

CPOM COMPLIANCE NOTICE
This position requires the selected physician to take authentic ownership and control of the Professional Corporation as required under California law. The PC-MSO structure governing this engagement is designed to ensure full compliance with the Corporate Practice of Medicine doctrine. Legal review of the MSO Agreement and Physician Services Agreement is encouraged prior to execution.
