Responsibilities
- Deliver direct patient and resident care, including assistance with activities of daily living such as bathing, dressing, mobility, and feeding.
- Monitor vital signs and observe changes in patient conditions, promptly reporting any concerns to nursing staff.
- Assist with medical procedures such as catheter care, medication administration, phlebotomy, and aseptic techniques.
- Support patients with mobility using equipment like Hoyer lifts and ensure safe transfer practices.
- Maintain accurate medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner while adhering to HIPAA regulations.
- Provide compassionate behavioral health support for individuals with dementia or Alzheimer's disease in memory or dementia care units.
- Collaborate with interdisciplinary teams in settings such as ICU, emergency departments, post-acute care facilities, or outpatient clinics to deliver comprehensive patient service.
Skills
- Proven experience working in diverse healthcare environments including long-term care, hospital medicine, ICU, emergency medicine, or behavioral health settings.
- Strong knowledge of medical terminology, physiology, anatomy, and infection control practices such as aseptic technique.
- Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for accurate documentation.
- Ability to perform vital signs assessment, medication administration, catheter care and basic life support procedures confidently.
- Experience working with individuals with disabilities or developmental delays; Alzheimer's and dementia care experience is highly valued.
- Skilled in patient transfer techniques using Hoyer lifts and other mobility aids; understanding of Level I/II trauma center protocols is a plus.
