Per Diem Hospice Aide (CHHA)

Job Summary
We are seeking a compassionate and dedicated Per Diem Hospice Aide to join our team. In this vital role, you will provide personalized, respectful care to patients in their homes or care facilities, supporting their comfort and dignity during challenging times. Your presence and assistance will help enhance quality of life for individuals receiving hospice services, ensuring they feel safe, cared for, and supported. This position offers flexible scheduling to accommodate your availability while making a meaningful difference in patients’ lives.

Responsibilities

  • Perform patient observation to monitor health status and report changes promptly
  • Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Administer medications and treatments following medical documentation and HIPAA regulations
  • Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient care accurately
  • Support patients with dementia, Alzheimer's, memory care, and other cognitive impairments with patience and understanding
  • Conduct vital signs assessments such as blood pressure, pulse, temperature, and respiration rates
  • Assist with catheter care, wound care, and other specialized medical procedures as trained
  • Provide emotional support and companionship to patients and their families during hospice care
  • Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and meal preparation
  • Use medical terminology appropriately when documenting patient interactions and care activities
  • Follow all safety protocols including CPR certification requirements for emergency situations

Qualifications

  • Prior experience in nursing home settings, assisted living, home health, or long-term care environments preferred
  • Knowledge of hospice care principles, dementia/dementia-related behaviors, Alzheimer’s care, and memory care best practices
  • Experience working with individuals with developmental disabilities or disabilities in general is a plus
  • Strong understanding of medical terminology and documentation standards such as HIPAA compliance
  • Valid CPR certification required; additional certifications like catheter care or medication administration are advantageous
  • Ability to work independently while maintaining excellent patient observation skills
  • Compassionate demeanor with excellent communication skills to engage with patients and families effectively
