The Palliative Care Nurse Practitioner provides palliative care consultation visits in the community setting, including home, assisted living and skilled nursing facilities. Palliative care consultation visits may include serious illness conversations, goals of care discussions, advanced care planning/directives, symptom assessment and symptom management recommendations with an emphasis on patient goals and quality of life. The Palliative Nurse Practitioner coordinates with the patient’s health care team through planning and effective communication.

Responsibilities for Per Diem Palliative Care Nurse Practitioner:

Performs palliative care consultations, and follow up visits with accurate and timely documentation
- Performs Palliative Care follow up visits based on individualized care plan
- Collaborates regularly with attending physician(s) and Hospice and Palliative Medical Director
- Provides education on trajectory of advanced illness

Assists in care coordination to meet patient goals, clinical needs and symptom management
Qualifications for Per Diem Palliative Care Nurse Practitioner:

Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program

Current nursing licensure in State
-
Master's degree with a minimum of (1) year Nurse Practitioner experience and a minimum of (1) year hospice or palliative care experience preferred

Accreditation from an approved certifying body for advanced practice nursing as required by State
-
Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)

Excellent observation, verbal and written communication skills, problem solving skills

Proof of Flu vaccination
