Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health by observing vital signs, recognizing changes in condition, and reporting findings accurately
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Administer medications following care plans and ensure compliance with HIPAA regulations for patient privacy and confidentiality
- Operate medical equipment like Hoyer lifts or wheelchairs safely to support mobility and prevent injury
- Maintain detailed documentation of care provided, patient observations, and any incidents in accordance with healthcare standards
- Support clients during outings or community activities to promote social engagement and independence
Skills
- Proven experience in home health, assisted living, nursing homes, or group homes with a focus on senior or disability care
- Knowledge of medication administration, vital signs monitoring, and basic nursing procedures including CPR and first aid certification
- Familiarity with dementia care, Alzheimer’s care, memory care, hospice care, and working with individuals with developmental disabilities or autism spectrum disorders
- Strong ability to perform light housekeeping tasks such as cleaning and laundry to maintain a safe environment
- Excellent communication skills for effective patient observation and collaboration with healthcare teams or family members
- Physical stamina for heavy lifting and assisting clients with mobility challenges using equipment like Hoyer lifts or resident’s <PERSON> lifts
- Compassionate attitude combined with patience and adaptability to meet diverse client needs
