Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records in compliance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation
- Support medication administration following prescribed guidelines and care plans
- Use specialized equipment such as Hoyer lifts or wheelchairs to ensure safe transfers and prevent injury during mobility assistance
- Conduct patient observation to detect changes in condition or behavior, especially for those with dementia, Alzheimer’s, autism or developmental disabilities
- Maintain a clean and safe environment through regular cleaning tasks and adhering to infection control protocols
- Collaborate with healthcare professionals, social workers, and family members to develop and implement personalized care plans
- Provide emotional support and companionship to foster a positive atmosphere for clients in home health or long-term care settings
Requirements
- Proven experience working in assisted living, home health, nursing homes, or group homes; experience with dementia care or hospice is highly valued
- Certification in First Aid and CPR is required; additional training in Alzheimer's or memory care is a plus
- Knowledge of HIPAA regulations to ensure patient privacy and confidentiality
- Ability to lift heavy objects safely using proper techniques; physical stamina for prolonged standing or walking is essential
- Strong observational skills for patient monitoring and reporting changes accurately
- Experience working with individuals with disabilities or developmental challenges such as autism or other special needs
- Excellent communication skills to coordinate effectively with team members and family members
- Valid driver’s license preferred if transportation between care sites is necessary
