<Responsibilities>
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records in compliance with HIPAA regulations
- Support medication administration and help implement individualized care plans tailored to each client’s needs
- Prepare nutritious meals and assist with meal planning to promote healthy living
- Operate equipment such as Hoyer lifts or transfer devices safely to support mobility and prevent injury
- Maintain a clean and safe environment through regular cleaning, sanitation, and organization of the living space
- Provide emotional support and companionship to enhance social engagement and mental well-being
<Requirements>
- Prior experience in home health, assisted living, nursing home, or group home environments is highly preferred
- Knowledge of patient observation techniques, vital signs monitoring, and basic nursing skills such as CPR and first aid certification
- Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer’s and memory care), or hospice care is advantageous
- Ability to perform physical tasks including heavy lifting and assisting with transfers using equipment like Hoyer lifts
- Strong understanding of HIPAA regulations to ensure confidentiality and privacy of patient information
- Excellent communication skills for collaborating with healthcare teams, patients, families, and social workers
- Valid certifications in CPR and first aid; additional certifications related to dementia or hospice care are a plus
