Pharmacy Technician (Full Time)

POSITION SUMMARY

Under the supervision of the Pharmacy Manager and/or Clinical Staff Pharmacist the Pharmacy Technician (PT) II performs tasks in the Pharmacy Department necessary for optimum patient care and pharmaceutical service. The PT II must be a registered, certified pharmacy technician with the New Hampshire Board of Pharmacy and maintain this registration and certification. The PT II under the direction of the Pharmacy Manager and/or Clinical Staff Pharmacist performs all duties of the Pharmacy Technician I. In addition, the Pharmacy Technician II, under the supervision of the Pharmacist, participates in IV Sterile Compounding.

POSITION REQUIREMENTS

Education:

High School Graduate or Equivalent

Licensure/Certification(s):

Current National Certification as a Pharmacy Technician (recognized by the NH Board of Pharmacy).

State of NH Board of Pharmacy Registration within 15 days of employment.

BLS certification required within 60 days of employment.

Skills:

Shall not have been convicted of a drug related felony or admitted to sufficient facts to warrant such findings.

Proof of successful completion of a NH Board of Pharmacy recognized course on sterile compounding and 600 hours of experience in sterile compounding prior to employment required. If candidate is unable to provide this proof, they must complete, within 6 months of hire, the NH approved sterile compounding course and complete 600 hours of Pharmacist supervised sterile compounding time.

Work Experience:

Prior Hospital experience or Compounding Pharmacy experience required.

ESSENTIAL FUNCTIONS

PURCHASING AND RECEIVING:

1. Purchase medications and pharmaceutical supplies consistent with Pharmacy policy and procedure.

2. Under the direction of the Pharmacy Resource Technician, maintain the integrity of the 340B purchasing program.

3. Receive medications and pharmaceutical supplies according to pharmacy policy and procedure. Stock these items appropriately, including confirming that bar codes are readable.

4. Review the cost and NDC number of medications that are received and update the pharmacy system NDC and cost information.

5. For new items accurately add all necessary information to the master and/or facility inventory to utilize the medication (for employee prescriptions this must be added to the outpatient pharmacy system inventory). In addition add all necessary information to Connect-Rx and the pharmacy system integrated with AccuDose-Rx to utilize the medication.

6. Maintain the integrity of shelf labels (edit or replace incorrect item numbers). Follow all established inventory control processes within the Pharmacy.

STOCKING, REPLENISHING, DISPENSING:

1. Stocking or replenishing of an automated dispensing machine or other stock location.

2. Under the supervision of a hospital pharmacist accurately select and distribute (restock) in a timely manner medications and pharmaceutical supplies. Insure that medications and supplies that are restocked are within their expiration date and correctly labeled.

3. Restock medications in appropriate quantities (maintain established par levels).

4. Accurately unit dose medications under the supervision of a hospital pharmacist per NH Ph 703.03 (c).

5. Under the supervision of a hospital pharmacist fill cassettes (daily cart fill) or distribute medications that have been ordered for patients (ex. for medications only available from the central pharmacy or for stat use) accurately and in a timely manner.

6. Process medication recalls.

7. Audit daily medication charges (charge reconciliation). Debit or credit medication charges based on the charge reconciliation process. Accurately debit and credit medication charges received from ancillary departments and the clinics in a timely manner.

8. Inspect medications and pharmaceutical supplies kept within the central pharmacy and distributed throughout the hospital (drug dispensing machines, storage cabinets, emergency carts, RSI kits, floor stock, et al) for misbranding (outdates) and on a monthly basis. Under the direction of the Pharmacy Resource Technician maintain and update all forms and spreadsheets (paperwork) associated with this process. Complete monthly inspection reports for review and sign off by the Pharmacy Manager for floor stock areas according to NH pharmacy rule Ph 709.04 (e).

PROCESSING PRESCRIPTIONS AND ORDERS:

1. Processing of refill request orders;

2. Retrieving of prescription files, patient files and profiles and other such records pertaining to the practice of pharmacy;

3. Counting, weighing, measuring, pouring and reconstitution of prescription medication or stock legend drugs and controlled substances;

4. Accepting a new oral telephone order;

5. Accepting an oral refill authorization from a provider;

6. Communicating a prescription transfer for a non-control medication to or from another pharmacy that does not maintain a common database;

7. Communicating orally or in writing, any medical, therapeutic, clinical, or drug information, or any information recorded on a patient profile that does not require professional judgment;

8. Performing the data entry of a prescription or medication order into the computer without supervision;

9. The task of reducing to writing a prescription left on a recording or message line. Prescription order can only be deleted by pharmacist on duty;

TECHNOLOGY

1. Demonstrate and maintain core user competency with Connect-Rx and AccuDose-Rx.

2. Demonstrate and maintain end user competency with the pharmacy information system (Ex. maintenance of the master and facility inventory, order entry, patient profile look up, etc.).

3. Demonstrate and maintain end user competency with the pharmacy outpatient system.

4. Demonstrate and maintain super user competency with medication administration (bar coding).

STERILE COMPOUNDING:

1. Compound hazardous and non-hazardous sterile products under the supervision of a pharmacist.

2. Demonstrate and maintain competency in aseptic technique.

3. Demonstrate and maintain competency in garbing and degrading for hazardous and non-hazardous sterile compounding.

4. Demonstrate and maintain competency for the cleaning and disinfecting of hazardous and non-hazardous sterile compounding areas.

5. Understand the requirements for environmental monitoring of hazardous and non-hazardous sterile compounding areas and assist the pharmacists in meeting these requirements.

6. Ensure hazardous and non-hazardous sterile compounded products are accurately identified, measured, diluted and mixed under the supervision of a pharmacist.

7. Ensure proper labeling, storage, dispensing and distribution of hazardous and non-hazardous sterile compounded products.

8. Comply with all policies and procedures related to USP 797/800 and NH PH 404.05 applicable to the sterile compounding of hazardous and non-hazardous compounding.
