Responsibilities

What you’ll do

  • Evaluate patients and develop individualized physical therapy treatment plans
  • Provide skilled therapy services in patients’ homes
  • Re-evaluate progress and adjust care plans as needed
  • Collaborate with interdisciplinary care team members
  • Educate patients and families on treatment goals and techniques
  • Complete clinical documentation in accordance with agency standards

  • Hourly pay range: $39.25 – $60.84 (based on experience)

Qualifications

REQUIRED QUALIFICATIONS


  • Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
  • Graduate of an accredited school in Physical Therapy

PREFERRED QUALIFICATIONS


  • One year of experience as a PT in a home health care setting is preferred


Required Licensure/Certifications

  • Unencumbered PT license in VT and NH or compact.

  • Area of Interest:
