Physical Therapist

Team Care Rehab Inc. is looking for a PRN Physical Therapist to join our team & assist with Evaluations at our facilities across San Antonio. We primarily treat our geriatric population, though we do have a Pediatric facility our main office operates from. Each Evaluation billed will be paid at $95.00/per evaluation, with any treatments (not required) to be paid at $45.00/hr treatment. Currently, we average a minimum of 2 evals a week we are looking for assistance with, but as we grow our caseload at new facilities that number can be expected to increase. Please reach out with any availability, we are very flexible in terms of days/times worked.

We are seeking a Physical Therapist to join our Team and provide comprehensive care to our patients in SNF and AL facilities. The successful candidate will be responsible for assessing patient needs, developing treatment plans, and providing physical therapy services. The ideal candidate will have a passion for helping others and a commitment to providing the highest quality of care. Responsibilities:

• Develop and implement individualized treatment plans for geriatric patients.
• Evaluate and assess geriatric patients' physical abilities and limitations.
• Provide patient education on proper body mechanics, posture, and exercises.
• Perform manual therapy techniques to improve range of motion, strength, and balance.
• Develop home exercise programs for geriatric patients.
• Document patient progress in accordance with professional standards.
• Complete Evaluations in a timely manner when referrals are received.

Pay: $95.00 per hour
