Position Summary:
We are seeking a skilled and compassionate Physical Therapist (PT) to join our growing home health team. The ideal candidate will provide in-home physical therapy services to patients recovering from illness, surgery, or injury. You will develop individualized treatment plans, monitor progress, and collaborate closely with nurses, physicians, and caregivers to ensure optimal patient outcomes.

Responsibilities:

• Evaluate patients’ functional status and mobility needs in their home environment
• Develop and implement personalized therapy plans and measurable goals
• Provide hands-on physical therapy to improve strength, balance, and mobility
• Educate patients and families on exercises, safety, and home care techniques
• Coordinate with the interdisciplinary team to ensure cohesive patient care
• Maintain accurate and timely documentation in compliance with Medicare and agency standards

Qualifications:

• Current Utah Physical Therapist (PT) License (required)
• 1+ year of experience in home health or related setting preferred
• Knowledge of Medicare regulations and home health documentation standards
• Strong communication, organization, and time management skills
• Compassionate and patient-focused attitude
• Reliable transportation and valid driver’s license

Pay: $46.46 - $70.00 per hour
