Job Summary

The Physical Therapist consults with residents to evaluate their physical condition and symptoms, diagnose movement dysfunction, and develop individualized treatment plans. The therapist will implement therapeutic exercise programs and educate residents on techniques that promote mobility, strength, and independence.

Key Responsibilities

Resident Evaluation & Treatment

  • Evaluate and treat residents with musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits

  • Provide treatment only after receiving a written physician order

  • Develop individualized treatment plans to maximize resident mobility and functional independence

Resident Screening

  • Screen new residents within 24 hours of admission

  • Screen long-term residents upon significant change or nursing referral

Documentation

Complete and maintain all required therapy documentation including:

  • Initial Evaluations

  • Weekly Progress Notes

  • Monthly Recertification Forms

  • Discharge Summaries

  • Additional documentation required by the facility or Regional Manager

Collaboration & Meetings

  • Attend required meetings including:

    • Medicare meetings

    • Care plan meetings

    • Resident staffing meetings

    • Family meetings

  • Collaborate with the Regional Manager and interdisciplinary team to develop programs that help residents achieve their highest functional level

Compliance & Standards

  • Assist the facility in maintaining state and federal regulatory compliance

  • Follow infection control policies and procedures

  • Adhere to OBRA Resident Rights guidelines

Operational Duties

  • Manage therapy caseload scheduling

  • Monitor productivity and notify the Regional Manager when staffing support or additional screenings are needed

  • Provide in-service training and education for facility staff

  • Maintain professional conduct, communication, and appearance

  • Perform other duties as assigned by the PT Director or facility leadership

Supervision

The Physical Therapist reports directly to the Regional Manager, who oversees clinical performance, documentation quality, and compliance with professional standards.

The Physical Therapist Assistant (PTA) works under the supervision of the RPT and carries out treatment plans established by the Physical Therapist. All PTA documentation is reviewed and signed by the RPT.

Physical Demands

This position requires moderate to heavy physical activity including:

Lifting

  • Assist residents with transfers between bed, chair, and commode

  • Transfers may involve residents weighing over 100 lbs

  • Lift supplies up to 50 lbs to chest level and 10 lbs overhead

Push / Pull

  • Push or pull wheelchairs with residents weighing up to 300 lbs for distances up to 200 yards

Work Environment

The Physical Therapist may work in a variety of clinical and environmental conditions within the skilled nursing facility.

Qualifications

  • Current State Physical Therapist License

  • Knowledge of therapeutic rehabilitation techniques

  • Ability to read, write, and communicate effectively in English

  • Must complete a minimum of 12 hours of continuing education annually
