Qualifications:

• Must have a valid New York professional license
• Must have graduated from an accredited Physical Therapy college or university
• Must have experience in long-term care
• Excellent written and verbal communication skills
• Strong leadership qualities

Responsibilities:

• Develop and implement physical therapy programs that focus on rehabilitation
• Evaluate a resident's abilities, limitations, and needs to understand their specific physical challenges
• Regularly monitor the resident's progress and adjust the treatment plan as needed.
• Record and document patients care services
• Modify the treatment plan to ensure it remains effective and addresses the resident's changing needs.
• Collaborate with other team personnel to achieve well rounded-care
