Physical Therapist- Skilled Nursing

Pay: $40.00 - $55.00 per hour depending on experience

Shift: Full time Monday - Friday

Location: Citadel Post Acute - 5121 E Broadway Rd, Mesa, AZ 85206

Citadel Post Acute is looking for a Full-time Licensed Physical Therapist to work in our post-acute setting. This is a fast paced, dynamic setting with a varying case mix that will challenge and reward you. We have an energetic team that provides quality care for patients and enjoys having fun while providing therapy services.

This position is open to PTs at all experiences, new grads welcome. Full ADL suite and multiple gyms give access to specialized tools to rehab wide range of patients.

* Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.

* Assesses patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.

* Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
