We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team in a Skilled Nursing Facility (SNF) setting. This role offers the opportunity to work closely with post-acute and long-term care patients, helping them regain mobility, independence, and quality of life.

Key Responsibilities

• Evaluate patients and create personalized, goal-oriented treatment plans
• Deliver high-quality therapy to post-acute and long-term care patients
• Improve mobility, balance, strength, and functional independence
• Track, document, and adjust patient progress effectively
• Educate patients and caregivers on home exercise programs
• Collaborate with interdisciplinary healthcare teams to ensure optimal outcomes

Requirements

• Active California Physical Therapist License (required)
• BLS Certification (required)
• SNF/LTC experience preferred
• Excellent communication and teamwork skills
