PHYSICAL THERAPIST – NURSING HOME

Hillside Manor Rehabilitation and Extended Care Center is a 400 bed independently owned, skilled nursing facility located in Jamaica Estates, Queens, New York. Our facility has an outstanding reputation as a provider of excellent resident care, since 1975. Qualifications:

• New York State licensed Physical Therapist
• 2 years of experience in sub-acute/skilled nursing/long term care
• Knowledge of MDS, CMI, G Codes, Rehab Optima preferred
• Flexible, Team player, culturally sensitive, led by example attitude preferred

Responsibilities include:

• Evaluate, assess and document accurately patient’s physical impairments and set up plan of care accordingly within scope of physical therapy for improving functional status and quality of life
• Provide clinically sound physical therapy treatment
• Effectively communicate with patients, physicians, health professional and families
• Supervise and guide physical therapist assistants in their treatment and documentation
• Complies with federal, state, and local legal and professional requirements
