Physical Therapist (PT) Lead – 13 Week Contract
Location: Northbrook, IL 60062

• Position Type: 13-week contract

• Pay Range: $2,159 – $2,421 (weekly)

• Experience Required: 1+ year Home Health (HH) experience + HCHB (Homecare Homebase) experience required

• Territory Coverage: Chicago team & Lake Forest region

• Schedule: 12-hour day structure as needed (home health visits; schedule varies by patient census)

• Licensure: Must meet Illinois PT licensure requirements

Position Summary & Responsibilities:

• Provide in-home physical therapy evaluations and treatment plans

• Manage patient caseload across Chicago and Lake Forest territories

• Document visits and care plans using HCHB system

• Coordinate care with interdisciplinary home health team
