Position Overview

We are seeking a compassionate and skilled PRN Physical Therapist to deliver exceptional, patient-centered care. In this role, you will step in to support our dedicated in-house team, working directly with residents to improve their mobility, restore functional independence, and elevate their overall quality of life.

Key Responsibilities

• Evaluation & Assessment: Conduct comprehensive initial and ongoing evaluations to determine residents' physical capabilities and rehabilitation needs.
• Treatment Planning: Design and execute individualized, evidence-based therapy plans tailored to specific resident goals.
• Therapeutic Intervention: Utilize advanced techniques and targeted exercises to manage pain, improve movement, and restore function.
• Progress Tracking: Closely monitor clinical outcomes and dynamically adjust treatment plans as residents progress.
• Compliance & Documentation: Maintain precise, timely, and compliant electronic medical records in alignment with federal, state, and facility regulations.

Qualifications

• Licensure: Active and unrestricted Physical Therapist (PT) license in the state of Illinois.
