Full Job Description

Elliott Concierge Physical Therapy is selectively hiring a Physical Therapist to help grow our Chicago practice.

We provide premium, one-on-one physical therapy in the patient’s home for patients who want individualized care, convenience, and a higher level of communication than they typically receive in a traditional clinic setting.

This role is designed for a therapist who is clinically confident, independent, reliable, and an excellent communicator. The right clinician will enjoy treating one patient at a time, building strong patient relationships, and representing a growing concierge physical therapy brand in Chicago.

We are looking for the right therapist to represent the practice, not simply the first available applicant.

What Makes This Role Different

This is not a high-volume clinic role and it is not traditional home health.

Our model is built around one-on-one care, clinical autonomy, strong communication, and a premium patient experience.

ECPT handles patient acquisition, intake, payment logistics, and general practice support, allowing the therapist to focus primarily on patient care and communication.

This role is best suited for a therapist who wants to build a premium, flexible caseload over time rather than step immediately into a guaranteed full-time schedule.

For the right clinician, there may be an opportunity to grow into a larger role within the Chicago market as patient volume expands.

Service Area

Our initial Chicago growth is focused primarily on the North Side and nearby central neighborhoods, including:

Lincoln Park, Lakeview, Roscoe Village, North Center, Ravenswood, Lincoln Square, Andersonville, Wicker Park, Bucktown, Logan Square, Gold Coast, River North, West Loop, and surrounding neighborhoods.

Therapists are not expected to cover all of Chicago. We are looking for clinicians who can clearly identify the neighborhoods they are comfortable serving and accept visits that fit their location, availability, and schedule.

We work to match patients within the therapist’s preferred service area whenever possible.

This Role May Be a Strong Fit If You:

Enjoy treating one patient at a time

Are comfortable working independently in patients’ homes

Communicate clearly and proactively with patients and families

Take ownership of the patient experience

Are reliable, punctual, and professional

Have strong clinical reasoning skills

Are comfortable managing your own schedule

Can clearly define the Chicago neighborhoods you are willing to serve

Have experience with orthopedic, post-surgical, balance, geriatric, or neurologic patients

Want flexibility but still take the work seriously

Are interested in growing with a premium, relationship-based physical therapy practice

This Role May Not Be the Best Fit If You:

Prefer a traditional clinic setting with a fixed daily schedule

Need a guaranteed full-time caseload immediately

Are uncomfortable traveling to patients’ homes or buildings within your selected service area

Prefer not to communicate directly with patients and families

Need frequent supervision or a highly structured clinic environment

Are looking for occasional visits without taking ownership of the patient experience

Responsibilities

Provide one-on-one physical therapy visits in the patient’s home

Evaluate and treat patients based on clinical need and your professional judgment

Develop individualized plans of care

Provide clear education to patients and families

Progress exercise, mobility, strength, balance, gait, and functional training as appropriate

Communicate professionally before and after accepted visits

Complete documentation in a timely manner

Represent Elliott Concierge Physical Therapy with professionalism, warmth, and reliability

Orthopedic injuries

Back, hip, knee, shoulder, and neck pain

Balance and fall prevention

Gait and mobility training

Strength and conditioning for older adults

Neurologic rehabilitation

General functional decline

Return to activity and wellness-focused care

Compensation

Compensation is typically $100-$125 per completed visit, depending on experience, visit type, geography, and clinical fit.

This is a 1099 contractor role designed for a flexible, premium in-home care model. Therapists provide their availability, identify the Chicago neighborhoods they are comfortable serving, and accept visits that fit their schedule and service area.

We respect the therapist’s time and maintain patient cancellation expectations to reduce avoidable schedule disruption.

Requirements

Licensed Physical Therapist in Illinois

Doctor of Physical Therapy or equivalent physical therapy degree

Professional liability insurance

Reliable transportation

Comfortable providing care in patients’ homes

Strong communication and documentation habits

Prior outpatient orthopedic, home health, geriatric, neurologic, or post-surgical experience preferred

Pay: $100.00 - $125.00 per hour
