Schedule: 20–40 hours/week, Monday–Friday

Duties:

· Evaluate each patient’s unique needs and develop personalized therapy plans that support safe, effective recovery

· Guide patients in restoring movement, strength, and function, helping them return to the activities that matter most

· Empower patients to reach their highest level of independence and performance, improving quality of life

· Collaborate with and support care team members to ensure a consistent, high-quality patient experience

· Contribute to a culture of excellence by participating in quality improvement efforts that enhance patient outcomes

· Maintain clear, accurate documentation to support continuity of care and informed clinical decision-making

· Uphold the highest standards of safety and infection control to protect patients and promote a healing environment

Requirements:

• Current State of Illinois Physical Therapy License
• Graduate of an accredited Physical Therapy program
• BCLS (CPR) certification
