Monday-Friday: Day Shift

Rotating weekend/holiday rotation for inpatient coverage
Patient Population: Primarily adult/geriatric general PT and orthopedics in outpatient setting. Occasional inpatient coverage during week dependent on staffing needs and inpatient census, with weekend/holiday inpatient rotation. Potential for pediatric inpatients depending on hospital admissions
Position Objective
The Physical Therapist is responsible for organizing, developing, and directing physical therapy services in accordance with the standards and practice of physical therapy, state and federal regulations, and Alomere Health’s policies and procedures. The Physical Therapist will provide high quality care, excellent customer service, fiscal responsibility, and maintain regulatory compliance at all times.
Essential Responsibilities
- Communicate with referring provider(s) regarding patient progress and needs
- Coordinate care with other professionals as needed to maximize patient outcomes
- Act as a resource to the area therapists regarding progression of facility provided protocols
- Appropriate supervision and/or delegation of tasks in accordance with state laws and scope of practice
- Adequate utilization of the electronic medical record including thorough research and retrieval of pertinent medical information
- Complete accurate and timely therapy documentation and billing
- Use of evidence based information and principles with application to therapy practice
- Provide safe, reliable, and efficient performance of appropriate therapy procedures to evaluate and treat patients across the lifespan
- Involve the patient and family (as appropriate) in discussion and development of the plan of care, expected outcomes and creation of patient specific functional goals
- Develop a plan of treatment for each patient based on initial and ongoing evaluation
- Evaluate mobility, strength, and current level of patient function, while setting goals to improve these areas that are scaled to patient abilities and realistically attainable
Other Responsibilities

Required Qualifications- Graduate from an accredited physical therapy program (P.T., M.P.T., or D.P.T.)
- Current (or eligible for) license from the MN board of Physical Therapy
- CPR certification
Preferred Qualifications
- One year post-graduate experience
- Electronic Medical Record experience
Knowledge, Skills and Abilities
- Knowledge of the role and scope of physical therapy services
- Ability to gather and critically analyze data to support rationale for appropriate patients evaluation and intervention
- Ability to observe and practice universal precautions, perform CPR, emergency first aide
- Ability to work in an environment that requires significant physical activity and mobility throughout the workday in a way that does not compromise patient or therapist safety
- Ability to work in multiple settings and travel as needed to perform the job
- Strong knowledge in the ability to utilize professional verbal, nonverbal, and written communication with patients, families, colleagues, and others
