Physical Therapist

Job Summary
Keystone Post Acute is a 65-bed skilled nursing facility providing short term and long term rehab and skilled nursing to the growing population in the area. Keystone Post Acute seeking a dedicated and skilled Physical Therapist (PT) to join our healthcare team in a full-time or part-time capacity. The ideal candidate will possess a strong foundation in physiotherapy principles, with experience across inpatient, outpatient, and home health settings. As a vital member of our multidisciplinary team, you will deliver comprehensive patient care, develop individualized care plans, and promote recovery and mobility for diverse patient populations. Your expertise will help improve quality of life through targeted therapeutic interventions and evidence-based practices.

Responsibilities

  • Assess patients’ physical conditions using clinical evaluation techniques, including physiology and anatomy knowledge, to determine appropriate treatment strategies.
  • Design and implement personalized care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and community care.
  • Administer treatments in various settings including hospital inpatient units, outpatient clinics, home health environments, and community facilities.
  • Collaborate with healthcare professionals to coordinate discharge planning and ensure seamless patient transitions across care settings.
  • Utilize electronic medical record (EMR) systems to document patient progress accurately and maintain comprehensive treatment records.
  • Educate patients and their families on exercises, injury prevention, and management strategies to support ongoing recovery at home or in the community.
  • Stay current with advancements in physiotherapy practices, medical terminology, and relevant clinical research to provide the highest standard of patient care.

Requirements

  • Valid licensure or certification as a licensed Physical Therapist with relevant regulatory bodies.
  • Skilled Nursing Facility experience is a plus!
  • Proven experience working in acute care hospitals, outpatient clinics, home health services, or inpatient rehabilitation units.
  • Strong knowledge of physiology, anatomy, medical terminology, and various physiotherapy modalities including trigger point therapy.
  • Experience developing care plans tailored to diverse populations such as geriatrics, pediatrics, orthopedics, and patients requiring discharge planning.
  • Familiarity with EMR systems for documentation and patient management purposes.
  • Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams.

Pay: $60.00 - $70.00 per hour
