A Skilled Nursing Facility in Orange, NJ is seeking a licensed Physical Therapist (PT) to provide rehabilitative services to short-term rehabilitation and long-term care residents. This is a full-time, permanent role requiring prior experience in a Skilled Nursing or post-acute setting.

The Physical Therapist will work collaboratively within an interdisciplinary team to support functional improvement, safe discharge planning, and regulatory-compliant documentation.

Key Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide therapy services to short-term rehab and long-term care residents
• Monitor patient progress and modify treatment plans as appropriate
• Maintain accurate, compliant documentation in accordance with Medicare and facility standards
• Participate in interdisciplinary care plan meetings and rounds
• Support discharge planning and functional goal achievement
• Ensure compliance with Medicare Part A, Part B, and payer documentation requirements

Required Qualifications

• Active New Jersey Physical Therapist license required
• Minimum 1+ year of experience in a Skilled Nursing Facility or post-acute rehab setting required
• Strong understanding of skilled documentation standards and Medicare guidelines
• Ability to manage a productive caseload in a SNF environment
• Strong communication and interdisciplinary collaboration skills
