AuthoraCare Collective is currently seeking a Physical Therapist Assistant (PTA) Case Manager Homecare/Long-term Care position for our Integrated Health Services (Skilled home-based-care/home health) department supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-Time: Monday - Friday 8:00am - 5:00pm.
The Physical Therapist Assistant provides services to restore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease under the direction of the Physical Therapist. The Physical Therapist Assistant adheres to the therapy plan of treatment for a case load of home health patients in collaboration with other members of the interdisciplinary team (IDT). Works with the Physical Therapist and Patient Physician to maintain a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance.
The Physical Therapist Assistant will establish and maintain a therapeutic relationship with patients, including providing community support for resources and education. Will dcument the goals and outcomes of care and brings experience and passion in working in an environment that follows AuthoraCare core values of humanity, compassion, empowerment, and excellence as well as inclusion of all team members, patients, and caregivers.
