Physical Therapist Assistant

AuthoraCare Collective is currently seeking a Physical Therapist Assistant (PTA) Case Manager Homecare/Long-term Care position for our Integrated Health Services (Skilled home-based-care/home health) department supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-Time: Monday - Friday 8:00am - 5:00pm.


The Physical Therapist Assistant provides services to restore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease under the direction of the Physical Therapist. The Physical Therapist Assistant adheres to the therapy plan of treatment for a case load of home health patients in collaboration with other members of the interdisciplinary team (IDT). Works with the Physical Therapist and Patient Physician to maintain a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance.


The Physical Therapist Assistant will establish and maintain a therapeutic relationship with patients, including providing community support for resources and education. Education and Experience
:

  • Associate degree in Physical Therapy from an American Physical Therapy Association.
  • Minimum of one-year physical therapy assistant experience home health or facility preferred.
  • Current and unrestricted Physical Therapy Assistant Licensure.
  • Current CPR certification.
  • Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Key Responsibilities:

Delivery of Patient Care

  • Directs and coordinates the care and services provided to patients in accordance with the patient’s Plan of Treatment based on all assessments.
  • Responsible for continuous assessment of patient/family needs and implementation of interdisciplinary Plan of Treatment.
  • Works with IDT to identify patient/family goals of care and establish and maintain the Plan of Care.
  • Establishes therapeutic relationship with patient/caregiver, including providing emotional support and teaching about disease progression and symptom management.
  • Recognizes influence of culture, ethics, and beliefs on clinical decision making.
  • Makes home visits in accordance with Plan of Treatment.
  • Communicates applicable changes to the Plan of Care to patient/family/facility as applicable.
  • Complies with infection control and exposure control policies.
