Physical Therapist Assistant

Aventura at Carriage Inn, located in Dayton, Ohio, is seeking a dedicated and compassionate Full-Time Physical Therapist Assistant (PTA) to join our rehabilitation team. This position offers a consistent 35-hour work week in a supportive, patient-centered skilled nursing environment.

Responsibilities:

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist

• Assist in implementing individualized treatment plans to improve mobility, strength, and functional independence

• Accurately document patient care and progress using Net Health

• Communicate effectively with the interdisciplinary team regarding patient status and progress

• Ensure compliance with all facility policies and regulatory standards

Qualifications:

• Active Physical Therapist Assistant (PTA) license in Ohio

• Experience in a skilled nursing or post-acute care setting preferred

• Experience with Net Health is a plus

• Strong clinical, communication, and organizational skills

• Commitment to high-quality, patient-centered care

Schedule:

• 35 hours per week

• Weekend or holiday rotation may be required
