Location: Hobe Sound, FL

A Skilled Nursing Facility in Hobe Sound, FL is seeking an experienced Physical Therapist Assistant (PTA) to join its rehabilitation team. This is a full-time, permanent position within a Skilled Nursing and post-acute care setting.

The PTA will work under the supervision of a licensed Physical Therapist to provide therapy services to short-term rehabilitation and long-term care residents.

Key Responsibilities

• Provide physical therapy treatments in accordance with established care plans
• Assist in implementing individualized treatment programs
• Monitor resident progress and report changes to the supervising Physical Therapist
• Maintain accurate and compliant documentation in accordance with Medicare guidelines
• Support safe discharge planning and functional goal progression
• Collaborate with nursing and interdisciplinary team members
• Ensure compliance with facility policies and regulatory standards

Required Qualifications

• Active Florida Physical Therapist Assistant (PTA) license required
• Minimum 1+ year of experience in a Skilled Nursing Facility or post-acute rehab setting required
• Strong understanding of Medicare Part A and Part B documentation standards
• Ability to manage a productive caseload in a SNF environment
• Strong communication and teamwork skills
