Physical Therapist (PT) – Skilled Nursing Facility

Location: Hobe Sound, FL

We’re currently seeking a Physical Therapist (PT) to join a well-run Skilled Nursing Facility in Hobe Sound, FL. This is a full-time, permanent opportunity for a therapist who is comfortable working in a SNF / long-term care setting and enjoys treating a medically complex, geriatric population.

Position Overview

The Physical Therapist will evaluate residents, develop individualized treatment plans, and deliver high-quality rehab services aimed at improving mobility, strength, balance, and overall functional independence.

Key Responsibilities

• Perform physical therapy evaluations and establish appropriate treatment plans
• Provide skilled therapy services to short-term rehab and long-term care residents
• Collaborate with interdisciplinary teams including nursing, OT, SLP, and physicians
• Document treatments and progress in accordance with facility and regulatory standards
• Educate residents and families on treatment goals and discharge planning
• Maintain compliance with Medicare and SNF documentation requirements

Qualifications

• Active Florida Physical Therapist license
• Experience in a Skilled Nursing Facility or Long-Term Care setting required
• Strong clinical judgment and time-management skills
• Ability to work independently while collaborating with a team
• Experience with geriatric and post-acute rehab populations preferred
