Physical Therapist for Home Health (Full-Time)

Bond County Health Department is seeking an experienced Physical Therapist to join their Home Health team. Providing therapy and rehabilitation services to patients in the home, the ideal candidate is a compassionate healer who is passionate about their patients and committed to helping them recover their independence and mobility. Bond County Home Health provides services in Bond, Clinton, Fayette, Madison, and Montgomery counties.

Essential Duties

• Working with the patient’s medical provider, plans a physical therapy program to restore function, relieve pain, prevent disability, and improve or maintain muscle function.
• Develops and administers the patient’s care plan and monitors progress.
• Oversees the work of the physical therapy assistant as well as collaborates with other health team personnel on the patient’s treatment, response, and progress.

Education/Qualifications

• Current Illinois physical therapy license
• Experience in the home health field preferred.
