Physical Therapist, HIGH RATES!!

The Physical Therapist (PT) provides physical therapy services to patients in a Skilled Nursing Facility to restore function, alleviate pain, and prevent disabilities. The PT collaborates with the rehabilitation team, physicians, and nursing staff to provide quality patient care, adhering to evidence-based practice guidelines and facility policies. We are looking for a full-time PT, but are open to considering part-time positions.

Essential Duties and Responsibilities:

• Implement individualized plans of care.

• Administer therapeutic interventions, including but not limited to:

  • Therapeutic exercise

  • Gait training

  • Balance training

  • Modalities (e.g., ultrasound, electrical stimulation)

  • Assistive device fitting and training

• Document patient progress, including progress notes, in accordance with facility, regulatory, and payer requirements.

• Collaborate with the interdisciplinary team.

• Provide patient and family education.

• Maintain accurate billing records.

• Adhere to facility policies and safety guidelines.

• Participate in continuing education.

• Maintain a clean work environment and ensure proper equipment function.

• Participate in quality improvement activities.

• Reports to the DOR.

Qualifications:

• Graduate of an accredited Physical Therapist program.

• Current state licensure as a Physical Therapist.

• Current CPR certification.

• Experience in a skilled nursing facility preferred.

• Basic Knowledge of Medicare and third-party payer regulations preferred.

• Strong clinical, communication, interpersonal, and organizational skills.

• Proficiency in electronic documentation systems.

• Ability to meet the physical demands of the position.

Preferred Skills/Abilities:

• Experience with NetHealth, PCC.

• Ability to speak Mandarin or Cantonese is a plus.
