Physical Therapist – Home Health (Contract)

Location: Oregon 97216
Pay: $2,351 – $2,635/week

Schedule:

• Day Shift: Monday – Friday, 8:00 AM – 5:00 PM

Position Summary & Responsibilities:

• Evaluate patients and develop individualized home treatment plans

• Provide therapy interventions to restore/improve physical, social, and mental function

• Establish and communicate care and discharge plans with patients/families and team

• Supervise therapy assistants as needed

• Complete accurate and timely clinical documentation

• Ensure patient safety and adhere to quality standards

• Perform other duties as assigned

Requirements:

• Doctor of Physical Therapy (DPT) or equivalent

• Active Physical Therapist license in Oregon

• BLS certification (AHA or equivalent)

• Valid driver’s license and at least 21 years of age
