Physical Therapist, Home Health

Home health therapy company looking for licensed Physical Therapists in the Conejo Valley areas for home based physical therapy evaluations, reassessments and discharges. Creating POC for patients, care coordinating and supervising/ co-signing for PTA's.

Flexible hours, great pay.

Must be organized, self-starter and reliable with good communication skills. Company uses electronic medical documentation, must be able to use computer/electronic devices.

This is an independent contractor/sole proprietor position; therapists must have or be willing to get EIN for W9 as well as valid credentials.

No relocation applicants please unless you are already planning to relocate to the area for other reasons.
