Job Summary
We are seeking a motivated and energetic Home Health Physical Therapist to join our dynamic team. In this role, you will provide comprehensive physiotherapy services to patients in their homes, promoting recovery, mobility, and independence.
Duties
- Develop and implement personalized care plans tailored to each patient’s needs, incorporating physiotherapy techniques such as trigger point therapy and orthopedics.
- Oversee PTA
- Conduct thorough assessments of patients’ physiology and anatomy to inform treatment strategies.
- Manage discharge planning processes to ensure smooth transitions from inpatient or outpatient settings to home care.
- Deliver patient care across various environments including home health visits, clinics, hospitals, and outpatient facilities.
- Collaborate with healthcare teams to coordinate care plans, ensuring comprehensive support for patients’ recovery goals.
- Utilize electronic medical record (EMR) systems accurately for documentation of assessments, treatments, and progress notes.
- Educate patients and their families on physiotherapy exercises, injury prevention, and health maintenance strategies.
Requirements
- Valid license or certification as a Physical Therapist in Colorado
- Proven experience in home & community care settings with knowledge of outpatient, inpatient, hospital, or clinic environments.
- Strong understanding of medical terminology, physiology, and anatomy related to musculoskeletal and neurological conditions in pediatrics
- Familiarity with KanTime a plus
- Excellent communication skills for patient education and interdisciplinary collaboration.
