Physical Therapist - PRN

Our Meadow Peak Rehab and Assisted Living (located in Taylorsville) is a skilled nursing facility operating 24/7. Our facility is committed to providing the highest quality of care and services assuring human dignity and quality of life for our residents, their families, and our employees.

JOB SUMMARY

Our Physical Therapists

This position is accountable to evaluate and treat all patients referred for Physical Therapy services, to assist in their return to optimal functional abilities, and to prevent additional disability. This includes communication with physicians and other health care personnel to assist in providing quality patient care.

Some of the job responsibilities may include but are not limited to:

• Ability to prioritize and coordinate the needs of multiple patients.
• Able to plan, develop, organize and implement successful Therapy plans.
• Ensure excellent communication among residents and families.
• Help create a culture of superior customer service.
• Assist with improving our current processes and PT Team

REQUIREMENTS

• SNF experience preferred
• Ability to work cooperatively as a team member
• Ability to be accurate, concise, and detail oriented
• Caring and compassionate
• Physical Abilities: Lift 50-100lbs

Licenses/Certifications: State License-Physical Therapist- Utah (Physical Therapist Registered)
