Ohman Family Living is hiring a Physical Therapist (PT) for our skilled nursing and long-term care facility. This is a great opportunity to work with a supportive team while making a direct impact on resident mobility, independence, and quality of life.

Physical Therapist Responsibilities

• Evaluate and treat residents in a long-term care / skilled nursing setting
• Develop individualized physical therapy treatment plans
• Improve mobility, strength, balance, and fall prevention
• Collaborate with nursing and interdisciplinary team members
• Complete documentation in accordance with Medicare and rehab guidelines
• Educate residents and families on therapy programs

Physical Therapist Requirements

• Licensed Physical Therapist (PT) in Ohio
• Experience in skilled nursing, rehab, or long-term care preferred
• Strong communication and clinical skills
• New grads welcome to apply
