Home Health Physical Therapist | PRN Opportunity

Your Role as a Physical Therapist

  • Deliver high-quality, patient-centered care aligned with Elara Caring’s mission
  • Assess patient needs, develop care plans, and monitor progress
  • Collaborate with interdisciplinary teams to optimize outcomes
  • Adjust treatment plans based on patient progress and goals
  • Supervise and guide support staff
  • Maintain accurate and timely clinical documentation
  • Promote physical therapy services within the community

Qualifications

  • Graduate of an APTA-accredited Physical Therapy program
  • Current, unrestricted PT license in the state of practice
  • At least 1 year of experience in a healthcare setting
  • Willingness and ability to travel within the service area (up to 50%)
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to lift, push, pull, or support up to 50 pounds

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
