What You'll Do

As a Home Health Physical Therapist, you will:

• Perform comprehensive initial evaluations to assess patient needs and develop individualized treatment plans.
• Design and implement therapeutic programs that improve mobility, restore function, relieve pain, and prevent further disability.
• Monitor patient progress and modify treatment plans to maximize outcomes.
• Educate patients and caregivers on exercises, safety, and strategies to promote independence.
• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure exceptional patient care.
• Maintain accurate, timely, and compliant clinical documentation.
• Follow infection control and safety guidelines during all patient interactions.
• Deliver compassionate, high-quality care that enhances patients' quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program
• Current, unrestricted Physical Therapist (PT) license in the state of practice
• CPR Certification
• Valid driver's license with reliable transportation and current auto insurance
• One (1) year of Home Health experience preferred
• HomeCare HomeBase experience preferred

Physical Requirements

• Ability to stand, walk, sit, bend, squat, kneel, and climb for extended periods.
• Ability to lift, carry, push, and pull up to 100 pounds with appropriate assistance when necessary.
• Ability to travel throughout the assigned service area to provide in-home patient care.
