Job Summary

Cypress Ridge Care Center is seeking a skilled and compassionate Physical Therapist to join our therapy team on a PRN or Part-Time basis. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide high-quality therapeutic care to support mobility, function, and overall quality of life.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans

• Provide direct physical therapy services in accordance with physician orders

• Document evaluations, progress notes, and discharge summaries accurately and timely

• Collaborate with interdisciplinary team members to ensure coordinated resident care

• Educate residents and caregivers on treatment plans, exercises, and safety techniques

• Maintain compliance with facility policies, state regulations, and professional standards

Qualifications

• Current Physical Therapist license in good standing

• Graduate of an accredited Physical Therapy program

• Experience in a skilled nursing or long-term care setting preferred

• Strong communication and documentation skills

• Ability to work independently and as part of a team
