Physical Therapist - Rogers/St. Michael Area

BrightStar Care of Plymouth is a small, but growing home care company, and we're looking for a Part-Time Physical Therapist (PT) to provide physical therapy in-home treatment for veterans. Join our team to make a difference for the individuals that served our country!

Having an active License on the Minnesota Board of Physical Therapy is required.

What you bring; Physical Therapist Must Haves:

• Bachelor's Degree or more in Physical Therapy from an accredited professional PT education program
• Active License on the Minnesota Board of Physical Therapy
• Current CPR Certification
• Ability to travel up to 20 miles as needed

Physical Therapist Duties:

• Provide physical therapy services to clients according to a physician’s written orders. Perform direct physical therapy treatment.
• Assess and evaluate therapeutic/rehabilitative/functional status.
• Develop a plan of care and treatment making sure plan of care is consistent with assessment.
• Initiate physical therapy program and instructs other personnel and/or family members and caregivers in specific phases of physical therapy with which they may assist with the client care.
• Participate in discharge planning, as appropriate.
• Report client’s response to treatment or changes in condition to the Director of Nursing and physician.
