Job Summary:

The Physical Therapist (PT) is responsible for evaluating, planning, and providing physical therapy services to pediatric, adolescent, adult, and geriatric patients with musculoskeletal, neurologic, developmental, and functional impairments. The PT develops individualized treatment plans focused on restoring function, improving mobility, reducing pain, preventing disability, and promoting overall wellness. The Physical Therapist collaborates with interdisciplinary team members and contributes to program development, patient education, and quality improvement initiatives.


Qualifications:


  • Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy required
  • Current Wisconsin and Minnesota Physical Therapist licensure required, or ability to obtain
  • Current CPR certification preferred
  • Minimum of one year of clinical experience preferred


Responsibilities include:


  • Perform physical therapy evaluations and provide treatment interventions according to established standards of care
  • Develop, implement, and modify individualized treatment plans based on patient needs, goals, and progress
  • Collaborate with providers, nursing staff, rehabilitation team members, patients, and caregivers to support optimal patient outcomes
  • Educate patients and families regarding diagnoses, treatment plans, home exercise programs, and safety recommendations
  • Supervise Physical Therapist Assistants and supportive personnel in accordance with state and regulatory guidelines
  • Complete accurate and timely documentation, billing, and communication in accordance with organizational and regulatory standards
  • Maintain equipment, infection prevention standards, required licensure, and continuing education requirements
  • Participate in departmental meetings, quality improvement activities, community education, and program development initiatives
  • Promote a professional, collaborative, and patient-centered environment consistent with OMC values

Knowledge, Skills, and Abilities:

  • Knowledge of physical therapy evaluation and treatment techniques across the lifespan
  • Strong communication, organizational, time management, and critical thinking skills
  • Ability to educate, motivate, and build rapport with patients and caregivers
  • Ability to work independently and collaboratively within an interdisciplinary team
  • Proficiency with electronic medical records and basic computer systems
  • Demonstrates professionalism, flexibility, sound judgment, confidentiality, and a positive attitude


Physical Requirements for the Role:


  • Frequent standing, walking, sitting, bending, reaching, squatting, kneeling, and repetitive hand use
  • Ability to lift, transfer, push, and pull up to 50 pounds occasionally and 20 pounds frequently with or without assistance
  • Frequent computer use and documentation throughout the workday
  • Ability to communicate effectively in verbal, written, and electronic formats
  • Ability to safely perform patient care activities in inpatient and outpatient settings


Work Environment:


The Physical Therapist provides services in both inpatient and outpatient settings. The work environment ranges from minimal to moderate noise levels and may involve exposure to patients with acute and chronic medical conditions, infectious diseases, and varying physical and cognitive needs.
