Job Summary:

We are seeking a dedicated, compassionate Physician Assistant-Certified (PA-C) to join our team full-time. This position offers a balanced schedule with patient care in a clinical setting and traveling home health visits, providing comprehensive care to diverse patient populations.

Salary Range

From $65 to $100 per hour (commensurate with experience levels and abilities)

Responsibilities
- Conduct patient assessments, diagnose illnesses, develop treatment plans, and prescribe medications in accordance with state regulations and collaborative agreements.
- Provide high-quality primary/urgent care in the clinic setting.
- Perform traveling home health care visits, including assessments, chronic disease management, follow-ups, and coordination of care for homebound patients.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes.
- Educate patients and families on health maintenance, disease prevention, and treatment plans.
- Document patient encounters accurately in electronic health records (EHR).
- Maintain compliance with all regulatory standards and participate in quality improvement initiatives.

Schedule

- Full-time: 5 days per week**
- 1 day in the clinic** (office-based patient care)
- 4 days traveling for home health care** (mobile visits to patients' homes; mileage reimbursement and travel time provided)

Qualifications

- A degree from an accredited Physician Assistant program

- 2 years plus experience
- Current certification as a Physician Assistant-Certified (PA-C) by the National Commission on Certification of Physician Assistants (NCCPA)** – required
- Active state licensure as a Physician Assistant (or eligibility)
- Valid driver's license, reliable vehicle, and clean driving record (required for home visits)
- Minimum of 1-2 years of clinical experience preferred; new graduates with strong home health interest considered
- BLS/ACLS certification
- Excellent communication skills, empathy, and ability to work independently during travel day(s)
