Position Summary:

This is a career-building opportunity for a Physician Assistant (PA-C) seeking a better balance between professional impact and personal life. In this role, you will provide on-site rounding care, manage chronic conditions, and build meaningful relationships with patients and care teams. You will have the autonomy to manage your day, grow your clinical expertise, and make a measurable difference in senior living settings.

Responsibilities:

Provide on-site rounding care in assisted living, memory care, and long-term care communities.
Perform assessments, chronic disease management, and preventative care.
Develop and adjust patient-centered care plans.
Collaborate with physicians, nurses, and interdisciplinary teams.
Communicate with families, caregivers, and facility staff.
Reduce hospitalizations through proactive care delivery.
Complete documentation, medication management, and lab coordination.

Qualifications:

Active Minnesota Physician Assistant (PA-C) license in good standing.
Experience in geriatrics, family medicine, or post-acute care preferred.
Strong clinical and communication skills.
Valid driver’s license and reliable transportation.
Passion for relationship-based senior care.

Navy: Physician Assistant (230X).
Air Force: 42G3 – Physician Assistant.
Marines: Physician Assistant (230X).
Coast Guard: Health Services Officer (HSO) / Physician Assistant.
