Job Summary
Join a mission-driven Federally Qualified Health Center serving communities across Benton and Linn Counties. The Community Health Centers of Benton and Linn Counties (CHC) provide integrated primary care, behavioral health, dental care, and substance use disorder treatment in a team-based, patient-centered setting. As the Physician Associate – Rapid Access Primary Care for Justice-Involved Persons, you will provide high-quality, trauma-informed primary care for individuals transitioning from incarceration to the community, while also supporting same-day and immediate access needs within the health center network. This role is designed to improve continuity of care, reduce barriers to timely treatment, and strengthen health outcomes for patients with complex medical, behavioral health, and social needs.
This is a full-time, benefited, limited duration position through the end of the biennium (June 2027), with the intent to seek ongoing funding in the next budget cycle.

About the role:

Provide rapid-access primary care for justice-involved individuals re-entering the community, with a focus on stabilization, continuity, prevention, chronic disease management, behavioral health support, and barrier reduction.

Conduct comprehensive medical assessments for incarcerated and recently released patients, including intake evaluations, acute visits, follow-up care, and transitional care planning.

Develop and manage individualized care plans that address chronic disease, preventive care, mental health needs, substance use disorders, and risk reduction.

Prescribe and manage medications within scope of practice, including treatment for substance use disorders such as Medication for Opioid Use Disorder, and continue indicated mental health treatment in collaboration with psychiatric and behavioral health partners.

Serve as a flexible rapid-access provider for CHC patients needing urgent or same day care across the clinic network.

Coordinate closely with custody, jail nursing, behavioral health, substance use disorder, case management, pharmacy, and primary care teams to support safe, effective, and integrated care.

Participate in interdisciplinary case review, peer review, quality improvement, and systems development efforts that improve care for justice-involved patients and reduce disparities.

Support trauma-informed discharge and transition planning, including linkages to primary care, behavioral health, medications, and community supports after release.

MINIMUM QUALIFICATIONSThe following minimum qualifications are REQUIRED for this position:-
Graduated of an accredited Physician Associate/Assistant program.
-
Current unencumbered Oregon license to practice as a Physician Associate/Assistant with prescriptive authority, or ability to obtain by date of hire.
-
National Commission on Certification of Physician Assistants (NCCPA) certified or eligible.
-
Current DEA registration, or ability to obtain by date of hire.
-
Current Basic Life Support (BLS) certification.
-
Ability to obtain and maintain credentialing and privileging in accordance with organizational standards.
Special Requirements:-
Hiring is contingent upon the successful completion of a background check.

Valid Oregon driver’s license or ability to obtain one by date of hire.
-
This position requires driving and may require use of a personal vehicle.
Preferred qualifications-
Experience in a community health center, FQHC, or other ambulatory care setting.

Experience in carceral health care, re-entry health care, or working with justice involved populations preferred
-
Experience providing care for patients with multiple barriers to health, including trauma history, housing instability, mental health conditions, substance use disorders, and other social drivers of health.

Ability to establish rapport and trust with patients experiencing trauma, severe and persistent mental illness, and/or substance use disorders. Motivational Interviewing experience is preferred.

Demonstrated ability to lead care teams and work respectfully and effectively with staff at all levels.
-
Strong documentation, coding, time management, and customer service skills, with comfort using the electronic health record during patient visits.

Bilingual English/Spanish skills are highly desirable.
