Position Summary:
As a Rounding Physician, you’ll be part of a collaborative team redefining primary care delivery in senior living. You’ll oversee a panel of residents, manage chronic and acute conditions, and guide care coordination across multiple disciplines. This role is ideal for a physician who finds fulfillment in developing long-term relationships and leading care that extends beyond traditional walls.

Key Responsibilities:

• Perform routine rounding visits across assisted living and memory care sites
• Oversee chronic disease management and preventive health initiatives
• Coordinate care across providers, staff, and family members
• Serve as clinical mentor and consultant for advanced practice providers
• Lead with compassion, ensuring dignity and clarity for each patient interaction

Qualifications:

• Active Minnesota MD or DO license
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• Prior experience in senior living or post-acute care highly valued
• Exceptional communication and leadership abilities
• Valid driver’s license and reliable transportation
