Position Summary:
We are seeking a Wound Care focused Nurse Practitioner to deliver hands-on, evidence-based wound management and primary care services within senior living communities. This role blends advanced wound care, chronic disease management, and mobile rounding, giving you the opportunity to practice at the top of your license. You will serve as a key clinical resource, driving healing outcomes, preventative care strategies, and interdisciplinary collaboration. Ideal for providers who thrive in autonomous, field-based roles and value long-term patient relationships.


Responsibilities:
  • Provide on-site rounding, wound care, and primary care services in assisted living, memory care, and skilled nursing settings.
  • Develop and manage advanced wound treatment plans, including pressure injuries, ulcers, and post-surgical wounds.
  • Drive chronic care management, preventative care, and care coordination.
  • Collaborate with physicians, nurses, therapists, and facility staff to optimize patient outcomes.
  • Reduce hospitalizations and ER visits through proactive clinical interventions.
  • Communicate with families and caregivers to ensure alignment on care plans.
  • Manage documentation, medication reconciliation, and lab coordination.


Qualifications:
  • Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
  • Experience in wound care, geriatrics, primary care, or post-acute care strongly preferred.
  • Valid driver’s license and ability to travel within assigned territory.
  • Strong clinical decision-making and independent practice mindset.
  • Passion for geriatric population health and relationship-based care.
