Work Schedule:

  • 3 x 12-hour shifts per week
  • 3 weekend shifts per month
  • Weekend shifts are included as part of the scheduled shifts (not extra)

Role:

  • We are looking for a compassionate Primary and Urgent Care Provider (MD, DO, APRN, NP, FNP, PA) to join our team of highly skilled clinical providers in locations across Connecticut.

Responsibilities:

  • Providing and managing immediate patient care, including physical examinations, evaluations, assessments, diagnoses, and annual well-visits.
  • Engaging with patients and their families or caregivers to understand and monitor self-management care plans.
  • Examining patients, evaluating medical histories, addressing patients' questions and concerns.
  • Ordering, performing, and interpreting diagnostic tests and explaining the results to patients.
  • Documenting individualized care plans and care coordination in the patient chart.
  • Analyzing patients’ medical history, medication allergies, physical condition, and examination to determine the best care.
  • Updating patients' charts and information to reflect current findings and treatments.
  • Prescribing pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Refer patients to specialists or to internal medicine as appropriate.
  • Directing and coordinating patient care activities of nursing and support staff.
  • Participating in meetings to evaluate practice processes and workflows pertaining to patient care and quality improvement efforts.
  • Participating in health promotion, education, and/or prevention programs, such as PCMH and ACO.
  • Following established policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.

Requirements:

  • Active medical licensure and ability to prescribe medications in CT.
  • Knowledge and experience in diagnosing and treating a wide variety of illnesses.
  • Primary Care and Urgent Care experience is a plus.

Supplemental pay:

  • Bonus structure available.

Pay: $120,000.00 - $250,000.00 per year
