Grin Grant's Recovery Advancement Center is an innovative integrated healthcare model designed to address the complex relationship between substance use disorders, chronic disease, oral health, behavioral health, employment, and long-term recovery.

Located in downtown Lexington, the Center integrates:

  • Primary care services
  • Medication-Assisted Treatment (MAT)
  • Dental services
  • Behavioral health services
  • Peer support and case management
  • Workforce development through BestKYTraining
  • Court-connected and justice-involved programming

The Center serves individuals across the full continuum of recovery, helping patients improve their health, overcome barriers, increase self-sufficiency, and reach their full potential.

Position Overview

Grin Grant's Recovery Advancement Center is seeking a compassionate, mission-driven Primary Care & Addiction Medicine Nurse Practitioner to provide integrated medical care and Medication-Assisted Treatment (MAT) services within a recovery-focused healthcare environment.

This is not a traditional primary care position.

The Nurse Practitioner will play a central role in helping individuals recover from substance use disorders while addressing chronic disease, preventive healthcare needs, oral-systemic health concerns, and barriers that impact long-term wellness and employment.

The successful candidate will work alongside behavioral health professionals, peer support specialists, case managers, dental providers, workforce development staff, and community partners to deliver coordinated, whole-person care.

Responsibilities Primary Care Services

  • Conduct comprehensive health assessments and physical examinations
  • Diagnose and treat acute and chronic medical conditions
  • Provide preventive care and wellness services
  • Order and interpret laboratory and diagnostic testing
  • Prescribe medications and develop treatment plans
  • Manage chronic diseases including diabetes, hypertension, hepatitis, cardiovascular disease, and other common conditions

Addiction Medicine & MAT Services

  • Conduct substance use disorder assessments
  • Evaluate patients for Medication-Assisted Treatment (MAT)
  • Initiate and manage buprenorphine treatment
  • Monitor treatment adherence and recovery progress
  • Develop individualized treatment plans
  • Coordinate care with behavioral health providers and peer support staff
  • Support individuals participating in treatment court, recovery housing, and other recovery-oriented programs
  • Utilize evidence-based practices in addiction medicine and recovery care

Integrated Care Responsibilities

  • Coordinate care with behavioral health providers, dental providers, and recovery support staff
  • Address health conditions that impact recovery, employment, and quality of life
  • Participate in multidisciplinary case reviews and care planning
  • Maintain accurate and timely clinical documentation
  • Participate in quality improvement initiatives and program development

Patient Population

The Recovery Advancement Center serves a diverse population, including:

  • Individuals receiving substance use disorder treatment
  • Individuals participating in Medication-Assisted Treatment (MAT)
  • Justice-involved individuals and court referrals
  • Residents of recovery housing and sober living programs
  • Individuals with chronic medical conditions
  • Patients experiencing significant oral health challenges affecting overall health and employability
  • Uninsured and underserved populations
  • Community members seeking accessible primary care services

Many patients face barriers that extend beyond healthcare alone. The successful candidate will be comfortable working within an integrated model that supports recovery, health restoration, workforce participation, and community reintegration.

Qualifications Required

  • Licensed Advanced Practice Registered Nurse (APRN) in Kentucky
  • Board-Certified Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
  • Active DEA registration
  • Ability to prescribe and manage controlled medications in accordance with applicable regulations
  • Strong clinical assessment and diagnostic skills

Preferred

  • Experience in primary care, addiction medicine, family practice, community health, or federally qualified health centers
  • Experience prescribing and managing buprenorphine treatment
  • Experience treating individuals with substance use disorders
  • Experience serving underserved populations
  • Knowledge of chronic disease management and preventive care
  • Interest in integrated healthcare and recovery-oriented systems of care
