Primary Care Nurse Practitioner / Physician Assistant

About Us: We are a patient-oriented private medical practice dedicated to providing exceptional primary care services to our Geriatric population. Our team of dedicated healthcare professionals is committed to delivering personalized and compassionate care to our patients. We are currently seeking a skilled and experienced Nurse Practitioner to join our dynamic team to round in assisted living and independent living facilities.

Job Description: As a Nurse Practitioner, you will play a crucial role in delivering high-quality primary care services to our patients. You will work closely with our physicians and healthcare team to diagnose, treat, and manage a wide range of medical conditions. Your primary responsibilities will include conducting patient assessments, developing treatment plans, prescribing medications, providing patient education and updating the families regarding any changes.

Key Responsibilities:

  • Conduct comprehensive patient assessments, including medical history and physical examinations
  • Develop and implement individualized treatment plans
  • Prescribe medications and other treatments as necessary
  • Order and interpret diagnostic tests
  • Provide patient education on health maintenance and disease prevention
  • Collaborate with physicians and other healthcare professionals to ensure continuity of care
  • Maintain accurate and detailed patient records
  • Participate in quality improvement initiatives and meetings

Qualifications:

  • Master's degree in Nursing/ Physician assistant from an accredited Nurse Practitioner/ PA program
  • Current Nurse Practitioner license in Illinois
  • At least two years of experience in a primary care/ geriatric setting
  • Strong clinical skills and knowledge of primary care practices
  • Proficiency in performing Pap smears, administering injections, and conducting IV infusions
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Job Type: Full-time

Pay: $100,000.00 - $130,000.00 per year
