Clinical Care & Rounding
- Perform routine rounding at assisted living and independent living facilities based on assigned schedule
- Manage an average daily census of ~15 patients per day
- Evaluate, diagnose, and treat geriatric patients with chronic conditions
- Respond to acute clinical issues and urgent calls from facility staff
Preventive & Longitudinal Care
- Complete Annual Wellness Visits (AWVs)
- Perform biannual care plan visits
- Conduct Transitional Care Management (TCM) visits following hospital discharges
- Ensure continuity of care across hospital, SNF, and assisted living settings
Care Coordination
- Communicate effectively with families and caregivers, especially during acute changes
- Collaborate with physicians, specialists, nursing staff, and facility teams
- Coordinate care plans and follow-ups to improve outcomes and reduce hospitalizations
Documentation & Compliance
- Maintain accurate and timely documentation in EMR
- Ensure compliance with Medicare guidelines and quality metrics
- Support initiatives focused on patient satisfaction, reduced length of stay, and readmission prevention
Job Type: Full-time
Pay: $130,000.00 - $140,000.00 per year
