Position Overview – Primary Care Physician
The Primary Care Physician (MD/DO) provides comprehensive, outcomes-focused outpatient care for older adult patients. You’ll build long-term relationships, manage chronic conditions, and deliver preventative care in a supportive, value-based clinic environment.
Responsibilities include:
- Diagnose, treat, and manage acute & chronic illnesses for a senior patient panel.
- Provide proactive health coaching and patient education.
- Lead a care team including care promoters, care facilitators, and coordinators.
- Collaborate with hospitalists, specialists, and transitional care teams.
- Take accountability for quality metrics: admissions, patient satisfaction, gap closures.
- Participate in clinic leadership, quality improvement, and physician recruitment.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO).
- Board Certified / Board Eligible in Internal Medicine, Family Medicine, or Geriatrics.
- Active state medical license (or ability to obtain).
- Current DEA license (Schedule II–V).
- Basic Life Support (BLS) certification within 90 days.
- Passion for value-based primary care and serving Medicare populations.

- Commitment to evidence-based medicine and continuous learning.
- Willingness to travel locally (clinic coverage) and attend national training (initial 2 weeks).
