This is a field-based role requiring travel to client homes across Dekalb County. Candidates must be comfortable working with our scheduling team and managing a flexible schedule.
Avant Home Care is seeking a reliable and experienced RN or LPN to perform supervisory visits and client assessments. This role is ideal for a nurse who values flexibility, autonomy, and making a direct impact in home care.
Responsibilities
- Conduct supervisory visits for caregivers in client homes
- Perform initial and ongoing client assessments
- Ensure services align with care plans and state requirements
- Document all visits accurately and in a timely manner
- Communicate updates, concerns, and changes in condition to the office
- Identify and report any compliance or care issues
Requirements
- Active RN or LPN license in Georgia
- Minimum 1 year of nursing experience (home care preferred)
- Strong assessment and documentation skills
- Reliable transportation and willingness to travel throughout service areas
- Ability to work independently and manage schedule efficiently
- Professional, dependable, and responsive
